patient. To this end we commit our total support in the recognition and
compliance with your expected rights as adopted by the American Hospital
Association and affirmed by the Medical Staff, Hospital Administration
and the Board of Directors. If you have concerns regarding patient care

YOU HAVE THE RIGHT TO:

receive considerate and respectful care regardless of sex, race,
cultural, economic, educational or religious background or the
source of payment for your health care;

obtain from your physician complete current information concerning
your diagnosis, treatment, and prognosis in terms that you can
understand;
-
to formulate Advance Directives and appoint a surrogate to make
healthcare decisions on your behalf to the extent permitted by law;
-
have your legally authorized representative exercise, to the extent
permitted by law, any healthcare decisions when you are found
incompetent or incapable of making your own decisions;
-
to participate actively in decisions regarding your medical care and
receive from your physician the information necessary to give
informed consent prior to the start of any procedure and/or
treatment;

to be informed by your physician or a delegate of your physician of
the continuing health care requirements following your discharge
from the hospital;
-
refuse treatment to the extent permitted by law, and to be informed
of the medical consequences of your action;
-
expect every consideration of your privacy concerning your own
medical care program and to expect that all communications and
records pertaining to your care will be treated as confidential;

expect that within its capacity the hospital must make reasonable
response to your requests for services;
-
expect reasonable continuity of care while a patient, during a
transfer or at discharge;
-
examine and receive an explanation of your bill regardless of source
of payment;
-
access the information contained in your medical records within the
limits of the law;
-
voice concerns and complaints regarding the quality of your health
care, including the convening of the Ethics Committee;
-
know what the hospital rules and regulations are that apply to your
conduct as a patient.
With
these rights there are responsibilities that you have to the
institution.
YOU
HAVE THE RESPONSIBILITY TO:

provide, to the best of your knowledge, accurate and complete
information in matters concerning your health;

make known whether you understand instructions given to you by
health professionals and to follow those instructions;

assure that the financial obligations of your health care are
fulfilled as promptly as possible;
-
be considerate of the rights of other patients, and hospital
personnel, and to assist in the control of noise, smoking and the
number of visitors;
-
notify receptionist or nurse if you require any communication or
assistive aids.
